Services
Crisis services are offered 24 hours per day in a residential placement. The
Crisis residence is located in Calvert county in Port Republic.
Residential Placement provides 24 hour supervision and direction. Staff gives guidance to consumers
in the areas of social and interpersonal skills, which include problem solving, communication, coping
methods, social and family relationships. Consumers along with staff support are required to
address the following areas: meal planning and preparation, grocery shopping, personal hygience,
house management, and time management. Consumers also receive education and support as needed
in budgeting, employment and medications. Staff provide consumers with crisis support and interventions.
The Assessment Process
Assessments initially include a psychiatric referral, a medical clearnace, and
a plan of care. Potential Crisis consumers are assessed by support staff at the time of the
referral application. A Community Network Plan, which assesses the consumers' needs,
is completed within the first 48 hours of admission. This assessment incorporates input
from the consumer, psychiatrist, therapist, physician and family, as appropriate.
A plan of action is agreed upon by support staff and consumer to assist the consumer
in stabalizing and returning to the community.
Special Needs
Crisis staff coordinate all services that the consumer receives with the consumer, and when appropriate,
with the consumer's natural support network, and the consumer's current medical and mental health
providers. Crisis staff link the consumer to all appropriate resources available in the community.
These linkages include charities, mental health services, medical services, hospitals, libraries,
social services, churches, drug and alcohol services, homeless outreach services, shelters and
other resources, as needed.
Consumers are provided with the support needed to effectively utilize their resources. This
includes, but is not limited to, transportation, assistance in making phone contacts, and
assistance in filling out applications.
