PROGRAM GOAL
Targeted Case Management (TCM) is a primary case management service for persons with mental illness. TCM ensures that persons with psychiatric disabilities have the resources to enhance their ability to function independently in their community. TCM provides services that are flexible, comprehensive, and mobile. Consumers and their families are provided many opportunities to establish peer networks and support systems.

- Linkages to community, psychiatric, legal, and medical resources
- Personalized and participatory goal planning
- Crisis Intervention and 24 hour emergency service
- Assistance with obtaining/locating transportation and housing
- Porcurement of psychiatric, financial and medical benefits
- Socialization and community integration
ELIGIBILITY
Targeted Case Management provides services to children ages 5-17 and adults 18 and over, who have a persistent mental illness, and meet medical necessity criteria, and do not have private insurance.
ADMISSION PROCESS
An initial intake will be completed to determine whether or not the individual's needs are consistent with TCM's medical necessity criteria. Once successfully completed, the individual will then proceed with the assessment package and individualized goal planning process, coordinated by our professionally trained staff.

The Targeted Case Management Program welcomes referrals from a wide variety of sources within the community. These include, but are not limited to:
- Self referrals
- Family Members
- Clinics
- Hospitals
- Private practicioner
- Jails
- Judicial Systems
- Homeless Shelters
- Department of Social Services
- Schools
