PSYCHIATRIC REHABILITATION PROGRAM (PRP)

The Psychiatric Rehabilitation Program services are offered on an individual, flexible, around the clock basis.  The services are consumer-centered and offered on a drop in basis in the consumer's home.  Individuals who are eligible based on diagnosis and financial criteria work with a Community Support Specialist to choose a program which will help them meet the goal of preventing institutionalization, and living an active and productive life within the community.  Services offered include:  linkage to community resources; linkage to psychiatrists and therapists; help with life skills; help with housing searches; financial management/budget assistance; and social skills education.  Recovery is the ultimate goal of the program, PRP assists individuals in meeting their definitions of recovery.  To qualify, you must be diagnosed with a major mental illness and have either Medical Assistance or otherwise qualify for the public mental health system in the state of Maryland.  Adults who receive PRP services will receive an average of two visits per week based upon the standards set by the State of Maryland.  To obtain services you may request a referral by contacting the PRP Coordinator:  410-535-4787 x. 318 (or come by our main office).

 

RESIDENTIAL PROGRAMS

The goal of the Residential Program is to provide supervised housing to individuals while they are working on their mental health goals.  The program provides the following types of housing support services:  assistance with developing daily living skills; assistance with developing self-care skills; providing links to community resources; developing a network of natural supports; and providing necessary and appropriate housing support.  Individuals must qualify on the basis of psychiatric need and must be eligible for the public mental health system.  SMCN has two levels of housing which include:  Intensive Level with 24/7 staffing by credentialed Residential Support Specialists and General Level housing where residents receive flexible drop-in support.  The level of housing is determined by the individuals clinical need, availability and their personal choice.  The Residential Program provides services to adults living in Calvert County who are diagnosed with severe and persistent mental illness.  For more information on this program or to obtain a referral, please contact our Residential Director at:  410-535-4787 x. 307.

 

TARGETED CASE MANAGEMENT (TCM)

The Targeted Case Management Program provides case management services for children, adolescents, and adults.  TCM ensures that persons with psychiatric disabilities have the resources they need to enhance their ability to function independently in their community.  TCM provides services that are flexible, comprehensive and mobile.  Consumers and their families are provided many opportunities to establish peer networks and support systems.  TCM services provide the consumer with linkages to community, psychiatric, legal, and medical resources; personalized and participatory goal planning; crisis intervention and 24-hour emergency service; assistance with obtaining/locating transportation and housing; procurement of psychiatric, financial, and medical benefits; assistance with IEP's and socialization/community integration.  TCM accepts referrals from a wide variety of sources within the community:  self referrals, family members, clinics, hospitals, jails, private practitioners, homeless shelters, the judicial system, schools, and the Department of Social Services.  The TCM program serves children ages 5-17 and adults 18 and over who have persistent mental illness, meed medical necessity criteria, and do not have private insurance.  For more information, or to obtain a referral, contact the TCM Program Director at:  410-535-4787 x. 313.

 

SUPPORTED EMPLOYMENT PROGRAM

The Supported Employment Program can introduce or re-introduce individuals with serious and persistent mental illness into paid employment and assist them in being successful.  The desired program outcome would be an eventual transition into full-time competitive employment.  For some individuals, steady, part-time employment might be a realistic goal.  Individuals engaging in full-time employment will eventually make the transition out of entitlement programs and into a fully financially independent status.  Persons who engage in part-time employment  would likely retain their benefits and participation in our programming though at a much more reduced level.  Many individuals due to the severity of their mental illness have difficulty obtaining and maintaining employment without consistent supports.  SMCN provides the supports needed for individuals to gain and maintain competitive meaningful employment.  The Supported Employment Program will assist individuals with coordination with other agencies and referrals; vocational assessments; vocational counseling and entitlements; job development; job coaching; and job placement.  Services are provided for adults at least 18 years of age, have a severe, persistent mental illness and are interested in employment, education, and training.  Individuals must also be eligible for the public mental health system.  For more information, or to obtain a referral, contact the Supported Employment Coordinator at:  410-535-4787 x. 328.

 

In-Home Intervention Program (IHIP)

IHIP is a program for children.  The program provides intensive in-home services to children, adolescents and their families to increase age appropriate skills and resiliency.  IHIP's goal is to reduce the risk of out-of-home placement while improving behavior and strengthening family functioning.  Children in the IHIP program will have an individual treatment plan which includes appropriate long-term and short-term goals that your IHIP case worker will help your child work on during home visits.  The program intensity is highest during the first 45 days and becomes less intensive thereafter as goals are reached.  IHIP is offered in both Calvert and St. Mary's counties.  IHIP consumers are between the ages of 5 and 17 years old.  Eligibility/participation for this program requires eligibility for Medical Assistance, diagnosis within the target population, and a psychiatrist/therapist referral.  To refer a child in Calvert County please contact the IHIP Calvert Coordinator at:  410-535-4787 x. 333; for St. Mary's County:  301-475-9315 x. 11; or for the Charles County Coordinator:  301-932-9146.

 

Child/Adolescent Psychiatric Rehabilitation Program (PRP)

After the IHIP program is complete or if a child has less intensive needs, the Child/Adolescent PRP program is offered.  The PRP offers services to children between the ages of 5 and 17 years old and follows the same eligibility criteria as the IHIP program listed above.  C/A PRP is geared toward regaining age appropriate skills, social and behavior needs, achieving school success, increasing family functioning and working on skills with the PRP program.  Each consumer will have a treatment plan with short-term and long-term rehabilitation goals that the PRP case worker will help your child work toward during the home visits and in the community.  PRP offers and average of one visit per week.  The C/A PRP services can be obtained in both Calvert County and St. Mary's County.  For the Calvert County Coordinator:  410-535-4787 x. 333 or St. Mary's:  301-475-9315 x. 11.

 

Adult Crisis Program

The Adult Crisis Program is designed to provide 24 hour/7 day a week assessment and stabilization to persons who are at risk for hospitalization or who need to step down from a hospital setting and can be expected to stabilize in a short period of time.  A typical length of stay at the Crisis house is 7 to 10 days.  The Adult Crisis house staff will always ensure that each individual is treated with respect, warmth, and dignity in an home-like environment.  When an individual may be in need of a Crisis bed a referral source must obtain the required information through a face-to-face evaluation with the consumer and document on the SMCN Adult Crisis Referral Packet.  A referral packet includes the referral application, statement of medical clearance, screening assessment, and mental status.  It is imperative that all required documentation is completed in full before an application can be considered for approval.  Services are provided for adults who are at least 18 years of age and reside in either Calvert, Charles, St. Mary's, Prince Georges, or Anne Arundel counties.  To refer a consumer who is in a potential crisis situation, please contact the Adult Crisis house at:  410-535-4892.
